Have ordered several replacement Speedo gears for my 93 YJ. Every time, the replacement speedo gear is a smaller physical size than to EOM and will not mesh with the speedo drive gear grommet in the Transfer Case. Do I need to replace the drive gear grommet in the transfer case as well?

You can’t have a range of 26-45 teeth and the gears be the same size. The speed sensor housing has tooth count sections on it. You have to rotate the speed sensor which is lopsided and will mesh the gear when you line up the correct tooth count with the mark on the transfer case. If you’re looking at the hole, the top side is 12:00, bottom is 6:00. The tooth count needs to be at the 5:00 position of the hole.
